This report pad,Daily Rating Scale, includes 6 preprogramed typical behaviors to rate, plus offers spaces for the teacher or student to write in additional goals. The simple preset rating scale will make it easy for busy teachers to provide the necessary feedback in a matter of minutes. Each pad includes 50 sheets and is 5 3/4" x 6 1/4".
