The bolster is placed under the knee during face up massage, under the ankle when face down, and can also be used to support the neck. Pay attention to the dimensions when considering this bolster. It is 'jumbo.' I wanted something bigger, and I thought with the flat size it would work well for under the ankles and knees. It does work well, but it is large. In fact, I have found it is too big to use with my 'regular' clients. I won't return it because it has a purpose. This bolster is great for clients coming in with moderate to severe back pain that need higher raised knees to alleviate the pain so I can work on them comfortably. It is really too large for ankles. The product description mentions for use on neck. I won't use it there. I couldn't imagine that being comfortable. So the smaller bolsters are a better choice, especially the fluffy bolster. But, if you are a therapist looking for extra cushioning to supplement and support a specific client base, this is your bolster. I also think it could be used well to sleep on your back if you need it to alleviate back pain...for non-therapists.
